Non-Invasive Management of Gynecologic Disorders

Aydin Arici and Emre Seli

There is an increasing trend within gynecology to move towards more "non-invasive" management techniques. These are less stressful for patients and reduce hospital stays and costs. As more of these modalities become available physicians need guidance in choosing among the variety of options. This book will concentrate on gynecologic disorders for which both surgical and non-surgical management options are currently available, and exclude gynecologic disorders that are treated only medically, or those that are managed mainly surgically (e.g. invasive cancers). The book will describe the pathogenesis of these disorders, and available non-surgical treatment options, but also offer guidance on how and when it is appropriate to choose more conservative modalities. An evidence-based approach will be followed throughout.
Sales projections are cautious because this is a new area; the intention is to establish this as the first book in the field, which will grow in importance through subsequent editions.

Modern Management of Abnormal Uterine Bleeding
Peter O’Donovan and Charles Miller

Menorrhagia - menstruation at regular cycle intervals but with excessive flow and duration - is one of the most common gynecologic complaints in contemporary women’s health. Surveys report that 30% of all pre-menopausal women perceive their menses to be excessive. The World Health Organization recently reported that 18 million women aged 30-55 years perceive their menstrual bleeding to be exorbitant.
Nearly 30% of all hysterectomies performed in the United States are performed to alleviate heavy menstrual bleeding. Definitive surgical correction has been the mainstay of treatment for menorrhagia; however, modern gynecology dictates a move toward conservative therapy both for cost-containment and because many women desire to preserve their uteruses. Alternatives to hysterectomy also are the result of statistics revealing that nearly 50% of uterine pathology findings from hysterectomies for menorrhagia are free of disease and histopathologic abnormalities. Many new forms of conservative therapy are now on the market, with more in development.
This major new book, written by an internationally recognized team of experts – many of them at the forefront of developing "second-generation" endometrial ablation techniques – covers the full range of surgical and medical treatments for menorrhagia. It will help gynecologists refine their operative techniques and consider new approaches to this highly challenging condition.

Atlas Urogynecological Endoscopy

Peter L. Dwyer

The study of gynaecology and pelvic floor disorders is a rapidly developing area for gynaecologists all around the world. An essential aspect to the diagnosis and treatment of pelvic floor disorders is a need for expertise in cystourethroscopy to:
Accurately diagnose conditions of the lower urinary tract that frequently present to the gynaecologists in the form of pelvic pain, bleeding or lower urinary tract symptoms of frequency, urgency or incontinence.
Endoscopy is also important for the gynaecologist to exclude damage of the urinary tract at the time of surgery which may be for stress incontinence, prolapse or other pelvic surgery e.g. hysterectomy, where there urinary tract may be at risk. This damage may involve the lower urinary tract by the inadvertent placement of sutures into the bladder wall or unrecognised injury to the bladder or urethra which can result in fistula formation. The upper urinary tract is also at risk during pelvic surgery by the inappropriate placement of sutures or clamps and can result in ureteric obstruction or fistula formation. Gynaecologists have little training or expertise in the use of cystourethroscopy and therefore do not feel comfortable in performing this procedure or distinguishing what is normal and what is abnormal. Increasingly, cystourethroscopy is becoming an essential part of the performance of surgery for stress incontinence and prolapse.
The aim of this book is to educate gynaecologists in endoscopy of the urinary tract to give them a good understanding of the equipment that is used, how it is used and the conditions of the lower urinary tract they may find in the treatment of pelvic disorders.
